Mini-App
What is it? ... The mini-app is a simple one-page Microsoft Word document used to collect basic financials prior to submitting for pre-approval on a loan. Also included below is the complete 1003 ("ten-oh-three"), the standard mortgage application. We normally only need the Mini-App to get everything started, but for us to move forward with a new loan or refinance we will eventually need the full 1003.
How do I download it? ... Right-click the link below and choose "Save Target As" and select a location on your computer (best is on your Desktop or in My Documents) where you will be able to open the file when your computer finishes downloading it. With it downloaded to your computer, double-click the file and it will open in Word. The document will ask for a password -- use "michael" to open it. Fill in all of the blocks and periodically go to File>Save to save your work (you may also click Ctrl-S to save). With the document filled in and saved, e-mail it back to us at: consult@fisherandwallace.com or print it out and fax it to us toll-free at 866-566-7244.

How do I use it? ... You can either download the form below, fill it out, and fax it back to us toll-free at 866-566-7244. Or you can follow the link to the Online Questionnaire below and simply submit it directly from our site. If you choose to download, follow the directions given above (under "Mini-App") on how to save the file to your computer.
